Mentally traveling back in time to my surgical training in New York, I remember:

gleaming BMWs and Mercedes Benzes, upscale shops, and weekday amblers dressed well enough for church...
My dreams of luxurious living quarters were short-lived, however.
The cabbie helped me lift my trunk filled with books from the cab.
In those moments, I felt naked, like a turtle dragging its shell across the sidewalk after losing its shell.
The people passed me like river water flowing around a boulder. For them, it seemed so natural.
They didn't even need to look at me.
I excused myself and rang the grimy doorbell through a pod of poor, dirty people.
When it opened, it revealed a man covered in tubes - one for each crevice of his face.
With each breath, I heard gurgling, bubbling sounds similar to drowning. I hesitated for a moment.
The huddle of bodies on the sidewalk made me want to step inside, but I also feared what lay within.
What if the accommodations were as eerie as the landlord?
As he stepped to the side, he gurgled, "Come in!"
I stepped into the darkness. I closed the door behind me, blocking out most of the street noise.
Do you feel hungry?" He bubbled. "I can prepare breakfast for you."
I took another closer look at the man and his condition, which was obviously post-surgical.
"No thanks, I'm fine."
"Are you certain?" He asked. "We include it in the rent."
"Perhaps tomorrow," I replied.
As I struggled to lift the trunk up the stairs, hoping that it wouldn't rip the carpet, he climbed ahead of me.
"I hope you weren't scared by the crowd outside."
"Uh, no, they're fine," I murmured.
He gestured toward a metal door on the other side of a curtain of bars and said,
"It turns out the city ran out of money to keep all the mental institutions running."
"Well, maybe I shouldn't call them lunatics," he said after adjusting a tube. Really, they're harmless.
There were many of them who had bad shakes or who got caught up in drugs."
"I see," I replied, trying to hide my horror.
The prospect of a room behind bars no longer sounded so bad.
The room was sparsely furnished with a wooden chair, a twin bed, and an oriental rug that had seen better days.
Breathing became difficult in the oppressive heat.
The city was experiencing its hottest summer in 80 years.
The lack of airflow inside the apartment made it even hotter.
Sitting in the wooden chair, I contemplated my next move. There was a growl in my stomach.
I felt my tongue clench against the roof of my mouth. Despite being tired, I was too hot, hungry,
and thirsty to sleep.
Normally, I would have eagerly explored the city to see what regional food it offered.
I was now concerned for my safety on the streets of New York City.
I looked out my grimy window pane at the people outside the front door, a story below.
Their speech was monotonous and broken. They shared cigarettes and scratched their noses viciously.
Their eyes darted in every direction. They seemed to be afraid of something...but I was afraid of them.
If I walked out there, would they attack me? Are they going to rob me? Or taunt me?
What about after I return? Would it be possible to get back in the house?
After all, they were numerous, and I was alone.
My mind struggled to comprehend the gruesome possibilities.
To distract my brain, I pulled out a medical book from the trunk and began reading.
My attention began to wane. I had to eat. The question is, how and where?
I felt my fear of the city grows again. My stomach growled.
There was no denying it - I needed food. After all, what options did I have?
Would I like to die under these miserable conditions?
So I repacked my belongings into the trunk and locked it, putting the key into my pants pocket,
along with the key to the front door the landlord gave me.
After deep breaths and pushing my shoulders back, I began to descend the stairs.
After I opened the front door, the people outside silently eyed me with sadness but then returned
to their half-hearted conversation, allowing me to pass.
My dinner consisted of roasted chicken and a tomato salad, which I devoured whole.
As I walked back to my room, I encountered no trouble. While I navigated around the crowd waiting to get
into the homeless shelter next to my new "home," I felt as though I did not even exist for them.
My studies took place six days a week at the Animal Medical Center in New York City, originally called
the New York Women's League for Animals. The organization's history as an anti-cruelty organization and
its modern reputation for cutting-edge veterinary care made the stay critical to my success.
I recently began building my own veterinary clinic in Switzerland with a business partner.
Our team struggled to determine who would take on new clients and who would handle emergency calls...
so instead, I went to New York. To gain ideas for our own Swiss practice, I would visit American veterinary practices
and see what equipment they used and how their clinics were built.
My partner would remain behind, managing our budding business.
Certainly, I would periodically check on progress in Switzerland, but for now, my focus had to be on
acquiring the necessary information and education to make our animal clinic a top-notch one.
I began each day by examining and caring for hospitalized patients at 7 a.m.
There was no lunchtime, other than chewing during exams and between patients.
It was usual for me to work until around 6:30 or 7 p.m., then attend classes until about 9 p.m.
Emergency services often followed classes at night.
I toured veterinary hospitals when I wasn't attending classes, participating in clinics, or studying.
On Sundays, when I had no classes, and most practices were closed, I treated myself to a large New York-style
pizza or a big fillet of bluefish with rice - a departure from my usual meals, which often consisted of salami,
cheese, or ham on a tortilla.
Eventually, I moved from the room that I was renting near the homeless shelter into a more modest
but better situated apartment I shared with another doctor.
When I had scrutinized every veterinary clinic in Manhattan's yellow pages,
I approached my trusted friend, Martin Q., who had been a radiology technician
at the Animal Medical Center for many years.
Having served in Vietnam and been injured, he studied veterinary medicine in Italy and
then moved back to the states, where I met him. Along with being well-traveled, he also had
a number of connections within the veterinary community.
He knew every New York City veterinarian by name.
Martin would surely be able to guide me.
Are there any good clinics you can recommend for me?" I asked him.
"A hospital with a high standard of care and well-equipped facilities?""
"Only the best," he smiled. "Doctor Martin De Angelis."
"Wow! You know him ?" I gasped. "He's a veterinary surgery rock star."
Do you want me to ask him if you can visit him?"
"Yes! Of course !" I shouted.
At first, I was jubilant - I couldn't contain my excitement at meeting Doctor De Angelis.
Soon, though, my spirits began to sink.
Surely, this was too good to be true. I thought it was unlikely to work out.
The weeks passed. In fact, I almost forgot about visiting the prestigious veterinarian...
and that's when it happened.
As suggested by a colleague, I would be able to get there most conveniently by train,
so I experienced Grand Central Station for the first time.
A flurry of activity and a vast number of bodies gave me a sense of how many people lived and worked there.
The train station was magnificent - it took my breath away.
Upon boarding the crowded train, I took in the scenery that rushed by like smeared watercolors.
Following Harlem, the train stopped in every neighborhood bearing its own discomforting signature.
The area was filled with homeless people and derelict homes.
My feelings at my arrival in the city welled up again, causing me to entertain the idea of staying on the train.
However, I also remembered the triumph of that first New York chicken and tomato salad.
While closing my eyes, I recalled how proud I had been of myself for bravely navigating the streets,
questionable characters, odors, and unusual sights.
Something had changed when I opened them. Was this something? Green?
During the train's route, it passed through ghettos and suburbs.
Gradually, the landscape became more open and inviting.
The vivid green of trees and grass soothed the spirit as if I had forgotten what open space looked like.
The countryside was stunning - a welcome relief from the dirty, hectic streets of the city.
As it turned out, I was not met by one of doctor De Angelis's technicians but by him, who met me at the door
and shook my hand.
"This way," he said. "Let's talk in my office."
We spoke about my aspirations for veterinary medicine back in Switzerland, my studies and experience,
and his practice for three or four hours.
He rose from his chair and said, "It's time for surgery." "How about scrubbing in?" he asked.
I'll start in twenty minutes."
My jaw dropped. Really? Did this world-famous doctor invite me to work alongside him?
I fought the urge to bow, replying, "I would be honored."
"But I have only performed a few basic surgical procedures."
I felt like a skiing student invited to ski with an Olympian on a black diamond slope.
I felt insecure. Shyness threatened to overwhelm me.
"Nonsense, let me show you a few things."
Show me a few things, he did.
As I left the Animal Medical Center in New York, the director Dr. Bill Kay explained to me that it had big donors.
Additionally, these affluent pet lovers traveled worldwide.
He asked if, when in Europe, any of these animal lovers' pets fell ill, would I agree that he refer them to me directly?
The words I had just heard were unbelievable.
The director of one of the world's best veterinary hospitals just asked me if I would accept the responsibility
of caring for the pets of the donors, the lifeline of his institution.
Of course, I said. What an honor...!
The level of expertise and friendliness I experienced during my stay in New York was unparalleled in my life.
Veterinarians in Switzerland are often (not all) guarded...unfriendly, and jealous.
The kindness and generosity I received here were beyond my wildest dreams.
As a result, my outlook changed. In Switzerland, I would have waited for this for years, decades,
or even forever. However, not in New York.
From my American experience, I brought everything I learned about protocols, facilities, equipment,
and procedures to my brand-new Swiss veterinary clinic.
I was expecting my very first child with my wife. It was perfect...except that my business partner and I couldn't agree.
Our visions for the clinic contrasted; we had trouble finding something in common.
Homecoming should have been a dream come true. In reality, it was a nightmare.
As my wife and I pulled away from the Swiss Post with our mail, we discussed how we would weather the storms.
Her eyes roamed over the pile of mail we had just retrieved from the post office.
She exclaimed, "It's a postcard from Doctor De Angelis.".
Suddenly, her voice changed pitch. "It asks, What are you doing? "I was looking forward to seeing you last week."
I ducked into a parking spot and in a phone booth nearby, tapped his number.

Two days later, my wife, unborn baby, dog, and I left for New York.
